LIFE Senior Care, a program designed to assist seniors with their customized healthcare needs, located in Jersey City Medical Center, is currently seeking a Full-Time LPN!

 

This position is full-time offering competitive compensation, a robust benefit package and generous paid time off.

 

General Responsibilities

 

The Licensed Practical Nurse performs various nursing related tasks that include assisting with participant assessments in the clinic or in the home and/or following an incident or during the performance of daily assigned duties and assists with implementation of the plan of care coordinated by interdisciplinary team.

 

Essential Duties

  • Supports the mission and values of Lutheran Social Ministries of New Jersey, which is: Through the power of the Holy Spirit, and in response to God’s love as revealed in the Gospel, the mission of the Lutheran Social Ministries of New Jersey is to serve those in New Jersey who hurt, are in need, or have limited choices.

  • Perform and/or assists with participant assessments in the clinic or in the home and/or following an incident or during the performance of daily assigned duties.

  • Assists with implementation of the plan of care coordinated by interdisciplinary team.

  • Administer and/or supervise the appropriate dispensing of participant medications excluding intravenous push medications.

  • Pre-fills medication boxes in the home per schedule and as needed.

  • Engages in teamwork and communicates effectively with the team.

  • Reports to Clinic Supervisor/Home Care Coordinator.

  •     Appropriately communicates participant condition with LIFE staff; assisting with supervision           of the Clinic CNA and Medical Assistant/Home Health Aides.

  • Other duties as assigned by my supervisor, necessary for the efficient operation of the department/facility.

  • Completes annual compliance and HIPAA training and exhibits behavior as set forth by the LSMNJ Code of Ethics and Business Integrity in the performance of duties.

  • Adheres to the Organization’s Policies and Procedures regarding the Compliance Program of the organization and is timely in reporting any possible concerns.

  • Follows all local, state and federal regulations as they pertain to the position.

  • Adheres to the Organization’s Policies and Procedures covering Privacy and Security in Compliance with HIPAA regulations.

  • Other duties as assigned by my supervisor, necessary for the efficient operation of the department/facility.

     

Experience and Skills:

 

 

Education/Training/Certifications:

  1. High school diploma
  2. LPN License in New Jersey
  3. BCLS

 

Skill(s): Skills and Knowledge:

                       

  1. 1 year of nursing experience in a hospital, nursing home or community-based setting.
  2. Broad knowledge base of health care problems of the frail elderly.
  3. Skilled in nursing assessments within scope of practice under the supervision of a Registered Nurse.
  4. Skilled in delegation and supervision of paraprofessional staff.
  5. Knowledge of common safety hazards and precautions to establish a safe work environment.
  6. Skilled in preparing and maintaining records and writing reports and responding to correspondence.
  7. Skilled in establishing and maintaining effective working relationships with participants, medical staff, staff members and family caregivers.
  8. Ability to react calmly and effectively in emergency situations.
  9. Current driver’s license and proof of auto insurance for home care.
  10. Knowledge of elder abuse and reporting
  11. Familiar with Dementia, Wandering Behavior and Intervention
  12. Knowledge of Standard Precautions & Fire and Emergency Procedures
  13. Knowledge of Participant Rights and Workplace Safety

 

 

Experience:  

        

  1. Minimum of 1 year working with the frail or elderly.
  2. 1-2 years experience as a Licensed Practical Nurse in a Skilled Nursing facility.
  3. 1 year of home health preferred for Home Care

 

Medical Clearance:

 

Employees must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
