LIFE Senior Care, a program designed to assist seniors with their customized healthcare needs, located in Jersey City Medical Center, is currently seeking a Per Diem Transportation Escort.

Through the power of the Holy Spirit and in response to God’s love as revealed in the Gospel, the mission of Lutheran Social Ministries of New Jersey is to serve those in New Jersey who hurt, who are in need, or who have limited choices.

General Responsibilities

Under the supervision of the Transportation Coordinator, the Transportation Escort is responsible for the safety and well being of participants while being transported in Lutheran Senior LIFE vehicles. 

 

 

Essential Duties

  • Supports the mission and values of Lutheran Social Ministries of New Jersey, which is: Through the power of the Holy Spirit, and in response to God’s love as revealed in the Gospel, the mission of the Lutheran Social Ministries of New Jersey is to serve those in New Jersey who hurt, are in need, or have limited choices.
  • Escort will assist driver in providing door to door services.

  • Escort is responsible for monitoring participants during transport on and off of Life vehicles, providing care in a manner which ensures participant satisfaction.

  • Transportation Escort must comply with all Lutheran Senior Healthcare Transportation policies and procedures in additional all general company policies. 

  • Transportation Escort must also assist in the transporting of potential participants, securing participants personal items, durable medical equipment, medications, and any other transportation related service as deemed necessary. 

  • Escort will assist in maintaining the cleanliness of the vehicle assigned on a daily basis. 

  • Escort is responsible for communicating any observed home safety and participant issues to the Transportation Coordinator. 

  • Responsible for monitoring Lutheran Senior Healthcare passengers during transport on Life vehicles, Follows assigned schedule makes adjustments to meet unanticipated changes, while considering the needs of the participant and requirements of the PACE Center.  Communicates these changes to the Transportation Coordinator or designee.

  • Immediately reports to Transportation Coordinator any vehicle or cell phone operating problems, malfunction, or incidents out of the ordinary routine.

  • Provides physical assistance to each participant when needed as she/he boards and disembarks from the program’s vehicles.

  • Communicates with Transportation Coordinator daily to be updated on participant conditions/changes, plan of care changes, or interdisciplinary team requests.

  • Follows Lutheran Senior Healthcare Policies and Procedures and OSHA safety guidelines.

  • Maintains the confidentiality of all company procedures, results, and information about participants, clients or families.

  • Handles potentially infectious fluids (e.g. blood, urine, and emesis) with appropriate biohazard precautions and practices Universal Precautions.

  • Participates in and supports Quality Improvement initiatives.

  • Participates in continuing education classes and any required staff and training meetings. 

  • Maintains any required certifications or licenses.

  • Must be able to work mandatory overtime for the efficient operation of the department/facility.

  • Completes annual compliance and HIPAA training and exhibits behavior as set forth by the LSMNJ Code of Ethics and Business Integrity in the performance of duties.

  • Adheres to the Organization’s Policies and Procedures regarding the Compliance Program of the organization and is timely in reporting any possible concerns.

  • Follows all local, state and federal regulations as they pertain to the position.

  • Adheres to the Organization’s Policies and Procedures covering Privacy and Security in Compliance with HIPAA regulations.

  • Performs other duties as assigned by my supervisor, necessary for the efficient operation of the department/facility.

 

Experience and Skills:

Basic Qualifications:

 Education/Training/Certifications:

  1. High School diploma or equivalent

  2. Experience with assisting the elderly or frail population

  3. Holds current MAVT and/or CPR certifications (preferred)

     

    Skill(s):

  1. Able to read and understand maps and routing information to navigate within the Hudson County area.

  2. Able to handle information in a confidential manner at all times. 

  3. Able to deal professionally and diplomatically with people. Must have good interpersonal skills.

  4. Experienced in transporting individuals with special mobility needs.

  5. Must be flexible and able to function in stressful situations.

  6. Requires individual who can coordinate multiple tasks simultaneously.

  7. Skilled in establishing and maintaining effective working relationships with participants, caregivers and co-workers.

  8. Proficient in identifying and recommending problem resolution.

  9. Ability to work independently with minimum supervision.

  10. Requires corrected vision and hearing to normal range, with or without reasonable accommodation.

     

    Experience:

    Minimum of 1 year working with the frail or elderly

     

    Competency:

    Position specific competencies for Transportation Escorts will be met prior to assuming participant care.

     

    Medical Clearance:

    Employees must have medical clearance for communicable diseases and up-to-date immunizations before having direct participants contact.
