Job Description:

SpiriCare, a home health care program, is currently seeking full time, part time and per diem Registered Nurses!! Under the direction and supervision of the Director of Nursing, the incumbent will be responsible for providing nursing care to SpiriCare participants in their homes. May perform in-home assessments and deliver skilled nursing in the home as needed. Also the RN will be responsible for implementation of physician orders and plans of care; delivering education to participants, caregivers and family; appropriately communicating participant condition with SpiriCare staff. Has direct over sight of Certified Home Health Aides in the home setting.

Duties include the following:

  1. Performing skilled nursing visits as needed.
  2. Engaging in team work and communicating effectively with the team.
  3. Updates LOC status sheets daily.
  4. Reports to the Director of Marketing weekly or more often as needed regarding LOC status.
  5. Completes annual Compliance and HIPAA training and exhibits behavior as set forth by the LSMNJ Code of Ethics and Business Integrity in the performance of duties.
  6. Adheres to the organization's Policies and Procedures regarding the Compliance Program of the organization and is timely in reporting any possible concerns.
  7. Follows all local, state and federal regulations as they pertain to the position.
  8. Adheres to the organization's Policies and Procedures regarding the Compliance Program of the organization and is timely in reporting any possible concerns.
  9. Follows all local, state and federal regulations as they pertain to the position.
  10. Adheres to the organization's Policies and Procedures covering Privacy and Security in Compliance with HIPAA regulations.
  11. Other duties assigned by the supervisor, necessary for the efficient operation of the department/facility.

Experience and Skills:

Education/Training/Certifications:

  1. Associate Degree required - BSN preferred
  2. Current New Jersey RN License
  3. Current BLS preferred
  4. Current driver’s license and proof of auto insurance
  5. Bilingual preferred

Skills:

  1. Broad knowledge base of health care problems of the frail elderly.
  2. Knowledge of common safety hazards in the home and improvements to establish a safer environment
  3. Skilled in identifying problems and recommending solutions.
  4. Knowledge of home care services nursing principles and practices with particular reference to the elderly.
  5. Ability to do physical assessments and develop plans of care.
  6. Knowledge of health counseling techniques and practices.
  7. Skilled in preparing and maintaining records and written reports and responding to correspondence.
  8. Excellent organization and documentation skills.
  9. Familiarity with community health resources, which are available to participants and families
  10. Skilled in establishing and maintaining effective working relationships with participants, medical staff, staff members and family caregivers.
  11. Ability to react calmly and effectively in emergency situations.

Experience:

  1. Minimum of 2 years of experience with the frail elderly population and completion of the Minimum Data Set "MDS" assessment form.
  2. 1 year nursing experience in a hospital, nursing home or community based setting.
