Job Description:

LIFE Senior Care, a program designed to assist seniors with their customized healthcare needs, located in Jersey City Medical Center, is currently seeking a Director of Nursing!

Under the direction of the Executive Director and the Director of Operations, is accountable for the direction, provision, and quality of 24 hour nursing services provided to Lutheran Senior Healthcare participants in the Center as well as in the home of participants. Responsibilities include development and implementation of nursing objectives, a nurse staffing plan, participation in the annual budget and financial objectives, and the development and implementation of a quality assurance plan for nursing services. Provides nursing services in accordance with New Jersey Board of Nursing requirements. Has knowledge of geriatric care issues and related nursing standards and creates a work environment that fosters teamwork and ongoing educational activities.

Essential Duties

  3. Assures development and maintenance of nursing standards, policies, and procedures based on established nursing knowledge and research in accordance with all applicable federal, state, local professional and regulatory requirements.
  4. Participates in the on-call process.
  5. Collaborates and communicates effectively with the Executive Director and other members of the management team.
  6. Works with other members of the interdisciplinary team to assure coordinated care that is high in quality, participant focused and cost effective.
  7. Demonstrates effective leadership in managing daily operations in nursing.
  8. Thoroughly and accurately evaluates performances of staff over the past year.
  9. Discusses evaluation and develops specific action plans to improve performance.
  10. Provides feedback throughout the year to help staff meet standards.
  11. Consistently plans for adequate staffing and space requirements. Ensures that staff are properly educated to perform assigned duties.
  12. Ensures accountability for and participation in the orientation process.
  13. Provides orientation, initial and annual competencies, supervises and evaluates performance of Home Care Coordinator, and Clinical Coordinator.
  14. Participates in the development of and competency evaluation of new employees as well as orientation activities.
  15. Performs competencies on appropriate Nursing staff as needed.
  16. Assures quarterly supervisory visits to staff in the field occur.
  17. Assures that staff maintain participants’ medical record and fulfills Lutheran Senior Healthcare charting and reporting requirements
  18. Assures that staff provide health counseling to participants and their families on an individual or group basis.
  19. Serves as a liaison between Lutheran Senior Healthcare and contracted Home Health services (as needed).
  20. Assists the Program Director in policy formulation and program development and implementation.
  21. In conjunction with the Home Care Coordinator and the Clinical Coordinator hires new staff and works collaboratively with the Human Resource Department to develop and amend job descriptions for nursing personnel.
  22. Develops policies and procedures which are reviewed annually and modified based on input from quality assurance activities.
  23. Controls and prioritizes utilization of nursing staffing needs to reflect actual enrollee needs.
  24. In conjunction with the Home Care Coordinator acts as the liaison between the contracted home health agency (if needed) and Lutheran Senior Healthcare.
  25. Maintains fiscal accountability for the provision of nursing services provided.
  26. Participates in developing and organizing new staff orientation, training and on-going in-services training for staff, independent contracts.
  27. Follows all Lutheran Senior Healthcare Policies and Procedures and OSHA safety guidelines.
  28. Maintains the confidentiality of all company procedures, results, and information about participants, clients or families.
  29. Handles potentially infectious specimens with appropriate biohazard precautions, and practices Universal precautions.
  30. Maintains safe working environment. Follows Lutheran Senior Healthcare’s Safety Policies and Procedures and Universal Precautions.
  31. Participates in and supports Quality Improvement initiatives.
  32. Participates in continuing education classes and any required staff and training meetings. Maintains professional affiliations and any required certifications.
  33. Completes annual compliance and HIPAA training and exhibits behavior as set forth by the LSMNJ Code of Ethics and Business Integrity in the performance of duties.
  34. Adheres to the Organization’s Policies and Procedures regarding the Compliance Program of the organization and is timely in reporting any possible concerns.
  35. Follows all local, state and federal regulations as they pertain to the position.
  36. Adheres to the Organization’s Policies and Procedures covering Privacy and Security in Compliance with HIPAA regulations.
  37. Other duties as assigned by my supervisor, necessary for the efficient operation of the department/facility.

Experience and Skills:

Basic Qualifications:

Education/Training/Certifications:

  1. MSN
  2. Nurse Practitioner License - New Jersey
  3. BCLS
  4. Current driver’s license and proof of auto insurance

Skill(s):

  1. Broad knowledge base of health care problems of the frail elderly.
  2. Knowledge of common safety hazards in the home and improvements to establish a safer environment
  3. Skilled in identifying problems and recommending solutions.
  4. Knowledge of home care services nursing principles and practices with particular reference to the elderly.
  5. Ability to do physical assessments and develop plans of care.
  6. Knowledge of health counseling techniques and practices.
  7. Skilled in preparing and maintaining records and written reports and responding to correspondence.
  8. Excellent organization and documentation skills. Previous management and leadership experience in Home Health Care
  9. Familiarity with community health resources, which are available to participants and families
  10. Skilled in developing and maintain department quality assurance. Knowledge of current NJ Home Health policies
  11. Skilled in establishing and maintaining effective working relationships with participants, medical staff, staff members and family caregivers.
  12. Ability to react calmly and effectively in emergency situations.

Experience:

  1. 5-years nurse management experience in a hospital, nursing home or community-based setting.
  2. Minimum of 1 year working with the frail or elderly

Medical Clearance:

Employees must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
