Journey Hospice is a new program of LSMNJ, and as with all Lutheran Social Ministries of New Jersey programs and services, Journey Hospice provides compassionate care to people—together. Journey Hospice serves people of all backgrounds, faiths, and ages experiencing loss and life-limiting illness. At this time, Journey Hospice provides its services in Atlantic, Cape May and Cumberland counties in New Jersey.

We are currently seeking a full-time Registered Nurse to work in Cape May County. This position offers competitive compensation, a robust benefit package and generous paid time off.

General Responsibilities: Under the supervision of the RN the LPN will perform revisits of  the Hospice patients and provides skilled professional nursing care to them, their families and caregivers as prescribed by the physician and in compliance with the Hospice Medicare Conditions of Participation, New Jersey Hospice licensure laws, accrediting body standards and agency policies and procedures. Provides the initial assessment and comprehensive nursing assessment and identification of problems of each patient; reassessment of nursing needs and collaborates to develop the Hospice Interdisciplinary Team (IDT) plan of care. Implements the plan of care and reevaluates and revises the plan of care relative to the patient’s nursing needs to provide appropriate, comprehensive and responsive goals and interventions. Teaches, supervises, and counsels the patient, family members, and staff regarding nursing care and the patient's needs, including other related problems of the patient at home. Works closely with other members of the IDT to ensure all patient and family care needs are being met. 

Experience and Skills:

Education/Training/Certifications: Graduate of an accredited LPN Program. Valid NJ LPN Licensure required.  Valid NJ Driver’s License with proof of insurance.

 

Skill(s): Ability to read and interpret documents such as safety rules, operating and maintenance instructions and procedure manuals. Ability to write reports and correspondence. Ability to speak effectively before groups of clients or employees related to Hospice philosophy.

Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardizations exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

 

Experience: Minimum one year experience in oncology and or/or hospice training program.
