Lutheran Crossings Enhanced Living, a senior care community located in desirable Moorestown (south Jersey), is currently seeking a Licensed Practical Nurse.

 

 

FULL TIME SECOND SHIFT OPENING

 

 

General Responsibilities

 

Supervises all licensed/certified professionals within the Assisted Living Facility in the performance of their daily assigned duties, adhering to all Local, State and Federal regulations.

 

Essential Duties

 

  1.   2. Directly supervises Assisted Living and *Residential Unit* staff during the performance of their duties, to include routine rounds.

  3. Keeps Administrator informed of any and all situations and changes that involve the day to day operations.

  4. Appropriately interacts with residents, responsible parties, family, friends, physicians, other related health care and non-health care professionals, visitors, vendors and Local, State and Federal regulatory agencies in a professional and courteous manner.

  5. Responsible for and/or assists staff with admissions, transfers and emergencies within the continuum of care.

  6. Delivers direct resident care within the scope of practice and consistently performs all nursing interventions required.

  7. Provides nursing care in conformance with established nursing standards and procedures.

  8. Documents all significant reported/observed medically related events or incidents within the resident’s file as outlined by all Local, State and Federal regulatory agencies.

  9. Reports emergent information to Resident Service Director in a timely manner. Shares in on-call status as needed.

  10. Relays pertinent information to all appropriate staff, physicians, responsible parties, licensing agencies as well as any other lawful authority as indicated by scope of event or incident as it relays to insuring the safety and well being if each resident.

  11. Assists with the completion of the resident’s plan of care, and annual MDS or approved assessment tool.

  12. Assists resident’s with activities of daily living, health related and psychosocial needs, serves as a health educator per resident need and/or request.

  13. Insure appropriate number of staff is available for each shift in the event of a call-in or “no show”.  Maintains staffing numbers within budget.  Provides counselling and discipline as needed.

  14. Follows all universal precautions; practice safe and proper handling and disposal of all infected materials, equipment and lines, follows established fire procedures and ensures staff compliance.

  15. Alerts Maintenance Director of any safety issues within the building.

  16. Maintains a safe and hazard free environment and report any such violation to Administrator/designee.

  17. Completes mandatory continuing education hours to ensure uninterupted licensure and certifications needed for job qualifications.

  18. Adheres to all LSMNJ/Grace Assisted Living policies and procedures.

  19. Attends all mandatory inservices.  Attends and/or leads QA programs.

  20. Assists with annual/ probationary nursing staff evaluations.

  21. Completes annual compliance and HIPAA training and exhibits behavior as set forth by the LSMNJ Code of Ethics and Business Integrity in the performance of duties.

  22. Adheres to the Organization’s Policies and Procedures regarding the Compliance Program of the organization and is timely in reporting any possible concerns.

  23. Follows all local, state and federal regulations as they pertain to the position.

  24. Adheres to the Organization’s Policies and Procedures covering Privacy and Security in Compliance with HIPAA regulations.

  25. Other duties as assigned by my supervisor, necessary for the efficient operation of the department/facility.

 

Experience and Skills:

Basic Qualifications

 

Education/Training/Certifications:

 High School Diploma/GED is required; graduate of an accredited school of practical nursing with current licensure by the NJ State Board of Nursing, CPR certified.

 

Skill(s):

Active listening and visual skills. 

Excellent verbal and written communication skills.

Strong time management skills.

 

Experience:  

Two years experience as a Licensed Practical Nurse; two years experience as a supervisor in a LTC health care facility and/or ALF is preferred.
